Student s are expected to demonstrate respect, integrity, and responsibility at all times.

Bullying, harassment, or any form of misconduct will not be tolerated and may result in disciplinary action, including dismissal from the program.

On full days student s will be expected to bring their own lunches. Snacks will be provided.

Regular attendance is crucial for the success of each student. Students are expected to at tend all scheduled classes and activities.

Parents/guardians must notify the academy of any absences in advance. Excessive absences may impact the student’s ability to participate in certain activities or programs.

I, the undersigned parent/guardian, give my consent for my child to participate in all activities related to the Leadership Academy, including  but not limited to classes, workshops, and field trips.

I understand that participation in the Leadership Academy involves  certain risks. I hereby release and hold harmless the academy, its staff,  and affiliates from any liability arising from my child’s participation.

I agree to assume all risks associated with the activities and understand  that the academy will take reasonable precautions to ensure a safe  environment.

I grant permission for the Leadership Academy to use photographs or  video footage of my child for promotional and educational purposes. This includes, but is not limited to, brochures, websites, and social media.

I understand that my child’s name will not be used without additional consent.

I authorize the Leadership Academy to arrange transportation for my  child to and from academy-related activities as necessary.

I understand that the academy will ensure that all transportation is  conducted safely and in accordance with applicable laws and regulations.

By signing below, I acknowledge that I have read, understood, and agree  to the terms and conditions outlined in this contract. I understand that  failure to comply with these terms may result in my child’s dismissal  from the Leadership Academy.
